I don't know about other places in Thailand, but I have been to Phuket four times. Each time I rented a car and never bought insurance. It's a left-hand drive, so I drive slowly and won't have any problems.
It is necessary because there are too many motorcycles in Thailand and they do not always follow traffic rules.
You must have full insurance. It's reassuring to have full insurance. Otherwise, if something happens, you'll be compensated to death.
