We rent a car in Porto, drive through Lisbon to Faro and then return it to Lisbon. The way there is mostly non-highway, and the way back is on the highway. Renting an ETC is convenient and fast.

No, I don't need it. Two years ago, when I went to the Western Portugal highway, there was a manned service for direct payment. Most of the time, there is no one on duty, and you can just swipe your credit card for payment.
