Just came back from Road trip in Kuala Lumpur. Now there is no cash toll on the expressway. It's all card payments. Just ask the car rental company for a Touch & Go card, and you can recharge it yourself at 7-11. There are many one-way streets in downtown Kuala Lumpur, so it's better to drive carefully. The drivers in Malaysia are very disciplined when driving, but you should be careful of the big buses, which are very powerful! ... But the expressway is really easy to drive on!
