No, there are lanes for both cash and card payments, and there are icons above the lanes. The highest fee I paid when traveling from Madrid to Lisbon was over 29 euros.

There are two ways to pay: one is to enter to get the card and then pay, and the other is to pay directly at the exit. Do not go through the T sign ETC entrance, go through the payment entrance with a credit card sign, both cash and credit cards are accepted. Do not go through the ETC channel with a T display, go through other self-service charging channels, which will show the price and accept both cash and credit cards.
