I just delivered it today. There's a list, but I can't find it now. I remember there are original and copies of passport (including visa page), original and copies of driver's license, ID card, one application form (Italian). I think that's it.

For certification, the original driver's license, the original passport with a Schengen visa, then make copies, print the translated documents. It is recommended to go for certification in Beijing, it's fast! If expedited, you can pick it up the next day!
