I got the Touch n go card at the car rental shop. You can also buy the Touch n go card at the petrol station, the leftmost lane of the highway toll, and some 7-Eleven or KK convenience stores. The deposit is 10 Malaysian ringgit. You can get the deposit back when you leave the car rental shop. If you want to return the card, it's a bit of a hassle. You have to return it at the service center in Kuala Lumpur. If you return the car at the airport, you can't return the card. You can only spend the money in the card. But the 10 Malaysian ringgit deposit cannot be returned. Talk to the car rental shop and ask them to return it to them. You can spend it at 7-Eleven or KK convenience stores.
