Excuse me, do I need to print out the insurance certificate when entering Rome, Italy ?

Do you mean the travel insurance you bought yourself or the insurance for the car rental?
All the capable ones should come out with it, just in case.
I can only say that I haven't encountered the situation I need to check
No checks were made when I entered the country.
I showed the one I had with me, but they didn't ask for it at immigration. I just got through immigration and waited in line for an hour.
Insurance policy not looked at, may be asked to see the itinerary or the first night accommodation. Generally ask you what you are doing here, when to go back. Italians are still pretty good at English.
No check, but it's best to print it out and bring it with you, better safe than sorry.
