Toll fee is collected by the car rental agency. The expressway fee is collected by the transportation department of the country where you are. (It's the expressway ticket stuck on the glass, bought at the gas station.) If you rent a car in the Czech Republic, you don't need to buy a Czech expressway ticket. Germany also does not need expressway fees.
