I'm in Milan on the 19th to the 29th, Rome out, Milan-Venice-Florence-Cinque Terre-Florence-Tuscany-Rome. Originally only rented a car between Florence and Rome. Just checked and it's 750 for Milan airport pick-up. Is it worth the four of us renting the car the whole time ?

Tuscany is a region, not a city. I recommend driving there.
From Malpensa Airport to the city center, take the light rail, 13 euros per person. The single ticket for Milan subway is 2 euros per person. The cost of renting a car is definitely higher than the bus fare, as well as the cost of refueling and parking. However, if you take a taxi every time you go out, and also arrange airport pick-up and drop-off services, it is a different matter.
