First of all, you need to get the car rental company's consent. Some car rental companies charge extra for adding drivers. Second, the Italian embassy seems to be quite particular about authentication documents.
I don't know if the rental car company installs any devices on the rented car. Even if not, once an accident happens, the insurance will be invalid if the registered driver is not the one driving.
